How to Enroll Your Child as PhilHealth Dependent
Prepare required documents
PSA birth certificate of child, your PhilHealth ID or MDR (Member Data Record), marriage certificate (for spouse enrollment), and valid ID.
Update your MDR (Member Data Record)
Log in to ePhilHealth portal (ePhilHealth.philhealth.gov.ph) and update your MDR to add the child as dependent. Or authorize a family member in the Philippines with an SPA to do this at any PhilHealth office.
Ensure contributions are current
Pay at least 3 months of contributions within the 6 months before any hospitalization. Pay through PhilHealth offices, accredited banks (BDO, BPI, Metrobank), or online (GCash, PayMaya, Bayad Center).
Keep a copy of the MDR
Print or save a digital copy of the updated MDR showing your child as dependent. The caregiver will need this at the hospital.
