Early Signs

Watch & Monitor

See your pediatrician same day — do not wait.

  • Persistent cough lasting more than 5 days
  • Low-grade fever (37.5–38.4°C) with cough
  • Slightly faster breathing than normal
  • Child seems more tired than usual
  • Mild loss of appetite with runny nose

Go to the ER Immediately If…

These signs mean your child needs oxygen and hospital care NOW — no exceptions.

Child is breathing very rapidly (over 60 breaths per minute in infants)

Lips, tongue, or fingertips turn blue or gray (cyanosis)

Skin between ribs or at throat pulls inward with each breath

Nostrils flaring with every breath

Child cannot be woken up or is extremely difficult to rouse

Breathing makes a grunt or wheeze sound

Child refuses all fluids for more than 4–6 hours

Fever over 40°C that doesn't respond to paracetamol

Age-Specific Guidance

Pneumonia by Child Age Group

Infants (0–12 months)

  • Any suspected pneumonia in newborns is a medical emergency
  • Rapid breathing (>60/min) or grunting sounds are key signs
  • Cannot tell you they feel chest pain — watch breathing carefully
  • High risk for severe complications — early hospitalization may be needed

Toddlers (1–3 years)

  • Often begins as a bad cold that worsens instead of improving
  • Check breathing rate when child is calm and sleeping
  • Watch for chest retractions — skin pulling in with each breath
  • Bacterial vs viral pneumonia requires different treatment — see doctor

School-Age (6–12 years)

  • May complain of chest pain, especially on deep breaths
  • Walking pneumonia (atypical) common — child may seem OK but needs treatment
  • Mycoplasma pneumoniae very common in school-age — mild but persistent
  • Ensure full antibiotic course is completed even if child feels better

Prevention

How to Prevent Pneumonia in Filipino Children

Pneumococcal Vaccine (PCV)

DOH-recommended PCV13 vaccine protects against the most common cause of bacterial pneumonia. 3 doses by 14 weeks.

Hib Vaccine

Haemophilus influenzae type b vaccine prevents a major cause of childhood pneumonia — included in the free DOH EPI program.

Breastfeeding

Exclusive breastfeeding for 6 months provides passive immunity that significantly reduces pneumonia risk in infants.

Clean Air at Home

Avoid indoor smoke from cooking and cigarettes — a major pneumonia trigger in Philippine households.

Handwashing

Frequent handwashing with soap prevents the viral infections that often lead to secondary bacterial pneumonia.

Good Nutrition

Adequate zinc, vitamin A, and protein intake strengthen immune defenses against respiratory pathogens.

Free Pneumococcal Vaccine Available in the Philippines

PCV13 is now part of the DOH Expanded Program on Immunization (EPI). Your child can receive it free at any Rural Health Unit (RHU) or government health center. View full vaccine schedule →
