What is Measles (Tigdas)?

Measles is a highly contagious viral infection caused by the Measles virus. It is airborne — one infected person can spread it to 9 to 18 unprotected people. It is the most contagious infectious disease known to science.

In the Philippines, measles is called tigdas and remains a significant child health threat. The 2019 PH measles outbreak was the worst in decades — nearly 47,000 cases and 632 deaths, mostly in children under 5. Despite a free, highly effective vaccine (MMR), measles continues to affect unvaccinated Filipino children.

Early Symptoms

Days 1–4

See your pediatrician immediately — contact tracing needed.

  • High fever (38.5–40.5°C) — typically abrupt onset
  • Cough — dry and persistent from early on
  • Runny nose (coryza) and red watery eyes (conjunctivitis)
  • Koplik's spots — tiny white spots inside the cheeks (pathognomonic)
  • General malaise, body aches, significant lethargy
  • Light sensitivity (photophobia) and painful eyes

Highest Risk
  • MMR vaccine is NOT given until 9 months — infants have no protection
  • Maternal immunity from vaccination wanes by 4–6 months

Pneumonia

Most Common

Most common severe complication, responsible for 60% of measles deaths in children. Risk is highest in malnourished and immunocompromised children.

Encephalitis

1 in 1,000 cases

Brain inflammation occurring in 1 in 1,000 measles cases. Can cause permanent intellectual disability, seizure disorders, or death.

Ear Infection

1 in 10 cases

Otitis media (middle ear infection) is the most common measles complication overall. Can lead to permanent hearing loss if not treated.

Blindness

Major in PH context

Corneal ulceration worsened by Vitamin A deficiency — a known risk factor in Filipino children. DOH Vitamin A programs reduce this risk.

SSPE

7–10 years later

Subacute Sclerosing Panencephalitis — a rare but fatal brain disease that can develop 7–10 years after measles infection, particularly in infants.

Immune Amnesia

2–3 years risk

Measles destroys immune memory cells, leaving the child vulnerable to other infections for 2–3 years after recovery — an underrecognized danger.

Prevention

How to Prevent Measles in Your Child

MMR Vaccine (Most Important)

Two doses of MMR vaccine provide 97% lifetime protection. First dose at 9 months, second at 12–15 months under the DOH EPI. Free at all barangay health centers.

Immediate Isolation

Isolate infected children immediately — measles spreads 9 days before rash appears. Keep home from school for minimum 4 days after rash onset.

Report to Health Authorities

Measles is a notifiable disease in the Philippines. Report to your barangay health center — contact tracing prevents outbreak spread.

Vitamin A Supplementation

WHO recommends Vitamin A for all children with measles. Reduces severity of illness and prevents eye complications. Ask your doctor for dosing by age.

Hand Hygiene & Mask

Measles spreads by airborne respiratory droplets — the most contagious way any illness spreads. Surgical mask for caregivers; N95 in hospital settings.

Post-Exposure Prophylaxis
