Is This an Asthma Attack? — By Severity Level

Identify the severity and take the correct action. Time matters during an asthma attack.

Mild Attack

Handle at Home

Give rescue inhaler (salbutamol). Monitor for 1 hour.

  • Mild wheezing — audible but child can talk normally
  • Slight shortness of breath during activity
  • Oxygen saturation 95%+ (if you have a pulse oximeter)
  • Child can walk and speak in full sentences
  • Responds well to 2–4 puffs of salbutamol inhaler within 20 minutes

Call 911 — Go to ER Immediately If…

Do not wait. These signs mean your child's airway is in danger.

Child cannot speak in sentences — only single words or unable to speak

Skin is pulling inward between ribs, at the neck, or below the breastbone

Lips, tongue, or fingernails turning blue or gray

No improvement after 3 rounds of rescue inhaler (every 20 minutes)

Child is drowsy, confused, or exhausted from the effort of breathing

Silent chest — no wheezing despite obvious breathing difficulty
