When Is a Fever an Emergency? Age-by-Age Guide for Filipino Parents

The same fever temperature means completely different things in a 2-month-old versus a 7-year-old. This age-by-age guide tells Filipino parents exactly when fever (lagnat) is an emergency — and when it is safe to manage at home.

  • Any fever 38.0°C or higher — always ER, no exceptions
  • Unusual crying or high-pitched cry
  • Difficulty feeding or refusing to feed
  • Bulging fontanelle (soft spot on head)
  • Skin color changes — pale, blue, or mottled
  • Breathing faster than 60 breaths per minute
Newborns cannot localize infection. A fever in this age group can mean sepsis (blood infection), meningitis, or other life-threatening conditions. There is no safe 'wait and see' approach.

3–6 Months

Tagalog: 3 hanggang 6 na buwan

  • Fever 38.0°C or higher — call doctor immediately
  • Fever lasting more than 24 hours
  • Extreme irritability or inconsolable crying
  • Difficulty breathing
  • Rash appearing with fever
  • No wet diapers for 8+ hours

Safe to Monitor at Home

  • Mild fever below 38.0°C with no other symptoms — monitor closely
Babies in this age group still have immature immune systems. Same-day evaluation is recommended for any fever. If you cannot reach your pediatrician within 2 hours, go to the ER.

6–24 Months

Tagalog: 6 buwan hanggang 2 taon

  • Fever above 39.0°C with no obvious cause
  • Febrile seizure (kejang)
  • Fever lasting more than 3 days
  • Rash with fever
  • Ear pulling with fever (possible ear infection)
  • Stiff neck

Safe to Monitor at Home

  • Fever below 39.0°C with clear cause (cold, teething) — paracetamol and monitor
  • Child is alert, drinking fluids, and playful despite fever
Febrile seizures are most common in this age group (2–5% of children). They are frightening but rarely dangerous. If a seizure occurs, place child on their side and time it. Go to ER if it lasts more than 5 minutes.

2–5 Years

Tagalog: 2 hanggang 5 taon

Go to ER / Call Doctor

  • Fever above 39.5°C not responding to paracetamol
  • Fever lasting more than 5 days
  • Severe headache or stiff neck
  • Difficulty breathing
  • Non-blanching rash (does not fade when pressed)
  • Dengue warning signs during rainy season

Safe to Monitor at Home

  • Fever below 39.5°C with clear viral cause — paracetamol, fluids, rest
  • Child is alert and drinking — monitor every 4 hours
  • Fever responding to paracetamol — continue home care
Most fevers in this age group are caused by viral infections and resolve in 3–5 days. During rainy season (June–November), always consider dengue if fever has no respiratory symptoms.

5–12 Years (School Age)

Tagalog: 5 hanggang 12 taon

  • Fever above 40.0°C not responding to medicine
  • Fever lasting more than 5 days
  • Severe headache with stiff neck (meningitis sign)
  • Difficulty breathing
  • Dengue warning signs
  • Extreme lethargy — cannot be woken

Safe to Monitor at Home

  • Fever below 40.0°C with clear cause — paracetamol or ibuprofen
  • Child is alert, eating and drinking — monitor at home
  • Fever from known viral illness — rest and fluids
School-age children handle fever better than younger children. Focus on comfort and hydration rather than the number on the thermometer. A child who is alert and drinking is generally safe to monitor at home.
